Our committed staff and volunteers will help clients identify their eligibility and which medications may be obtained free or at a low cost.

WHO WE SERVE

The Medication Access Program (MAP) provides assistance to qualifying persons of any age who are experiencing financial hardship and difficulty with paying for their medications. MAP services Portage, Columbiana, and Jefferson counties. While this program does tend to be geared more towards long-term medication needs (versus, for example, the need to obtain a short-term antibiotic), the staff and volunteers may also be able to point clients towards other resources in the local community. Additionally, individuals needing assistance in applying for Medicare Part D or with re-enrollment questions should also contact our offices.

Clients must bring the following to their appointment:

  • Medication bottles
  • Prescriptions and/or samples from your physician
  • All physician names, addresses, and phone numbers
  • Social security card, state-issued photo ID, and proof of birthdate
  • Proof of income for the current year
  • Current health insurance cards
