The Medication Access Program (MAP) provides assistance to qualifying persons of any age who are experiencing financial hardship and difficulty with paying for their medications. Our committed staff and volunteers will help clients identify their eligibility and which medications may be obtained free or at low cost. While this program does tend to be geared more for long-term medication needs (versus, for example, the need to obtain a short-term antibiotic), the staff and volunteers may also be able to point clients towards other resources in the local community. Additionally, individuals needing assistance in applying for Medicare Part D or with re-enrollment questions should also contact our offices.

Clients must bring the following to their appointment:
- Medication bottles
- Prescriptions and/or samples from your physician
- All physician names, addresses and phone numbers
- Social security card, state issued photo i.d. and proof of birthdate
- Proof of income for the current year
- Current health insurance cards
F&CS operates MAP in two counties, Columbiana and Portage. MAP will conduct in-person appointments with any individual, regardless of county of residence. In Portage County, those in need should call the MAP office at 330.297.7027 Ext. 332 to schedule an appointment. Appointments are currently conducted at our central office in Ravenna.
In Columbiana County, MAP operates at four sites throughout the county from 9:30 am to noon:
1st Wednesday: First United Methodist Church, 114 E Washington St. Lisbon
2nd Wednesday: Trinity United Presbyterian Church, 110 Main Blvd. East Liverpool
3rd Wednesday: First Christian Church, 831 Main St. Wellsville
4th Tuesday: First Christian Church 1151 East 6th St. Salem
